The Program Manager, a member of Social Work Services, acts as the family's link to the rehabilitation treatment team and facilitates the family's participation in the treatment program.
The Program Manager will:
- meet with you soon after admission to answer your questions and to conduct a psycho-social assessment.
- update the family regarding the patient's progress after weekly team meetings.
- links the family and patient with needed resources, which may include temporary disability funds, Social Security, Medicaid programs, home care services
- provide family support as the family adjusts with the stress and anxiety during this time of difficult changes, which accompany brain injury.
- facilitate the monthly NJ Brain Injury Support Group for Mercer County area residents who are survivors and caregivers.
