A comprehensive evaluation is the first step when a patient arrives at St. Lawrence. This includes a thorough assessment of the patient’s current status as well as a review of his or her previous functional status, lifestyle and support system. The impairment and strengths revealed during this evaluation as well as patient and family input, enable the rehab team to set measurable and meaningful goals.
Strong lines of communication between team members is standard practice at St. Lawrence. Through both informal and formal communication we keep each other updated on each patient’s progress. This enables us to quickly address areas of concern and to adjust the rehab strategy as needed. In all of our communication we are always mindful of the patient’s status as the key member of the team.
We include the family. Patient/family meetings with members of the rehab team are scheduled prior to discharge and as needed during the patient’s stay. These educational sessions are another means for ensuring open communication, reviewing progress and promoting a safe and smooth transition home.
